Registered Nurse
Job Description:
Provide nursing and a variety of PACT services to consumers.
Assist the PACT physician, and assist with management of the medication protocols.
Provide services to a caseload of PACT consumers and work closely with other members of the PACT team.
Perform nursing duties to include, but not limited to assessing/checking-in consumers in preparation for physician visits, administration of oral and IM medications, transcription of physician orders, ordering and packaging medications, and providing education/counseling regarding medications and relevant nursing problems.
Assist in the provision of medical services, such as delivering and monitoring medications, scheduling and accompanying consumers to medical appointments, collaboration with medical providers and requesting medical records
