Licensed Practical Nurse
Job Description:
Responsible for contributing to the biological, psychological and social assessment of clients.
Assist with the development and implementation of health related components of the Individual Program Plan (IPP).
Collaborate with members of the Interdisciplinary Team (IDT) and other health care professionals in the community.
Within the scope of practice the nurse provides technical support to non-licensed staff regarding health care services and medication administration.
At the direction of the Registered Nurse (RN) Supervisor the LPN, within the scope of their practice, will assess, implement and evaluate the individual’s healthcare needs for all acute and chronic healthcare conditions.
